Confused About PF and Pension Withdrawals? Need Help Choosing the Right Option!

Hi all,

While submitting for PF, I am getting two options:
a) Only PF Withdrawal; and
b) Only Pension withdrawal.

If I select the option for Only Withdrawal, can I get the pension amount, or should I apply for both individually? I want to withdraw both PF amount and pension amount. Can anyone suggest? Please find attached the document for your reference.
