If you have 3 major reportable accidents and 12 first aid cases in a year, then there must be many more near misses, and your work environment needs an audit. In any work environment, only 1/3 is visible/reportable accidents, and 2/3 are near misses, which happen every day. The graph may go up at any time. You have to observe the work environment, systems, procedures, and behavioral patterns of each employee carefully for correction. Since the number is small, proper safety procedures can be put in place at the initial stage itself. Do not be complacent with your 1/3 cases.
