Pl peruse the Payment of Wages Rules applicable to your state, all the forms and registers to be maintained under the PWA is mentioned therein.

Labour Welfare Fund legislation is a state statute and there is no central statute with pan India jurisdiction. So you will have to check up with respective state law on the deduction to be made.
