What is the basic structure to issue a payslip for contract employees?

Hi seniors, We have few contract employee in our IT firm. What is the basic structure to issue a payslip for them.Kindly suggest a format or template on the same

You may direct the contractor to comply with the requirement of Form T under the T.N. Shop & establishment Act 1947. I have extracted a copy from net, you may get it verified also.
