Starting a Performance Appraisal System from Scratch in an Import/Export Company: Need Help with Calculations?

I am working in an import/export company and need to formulate a performance appraisal. Could you kindly guide me on how to start? Previously, there was no appraisal system in place, so I have to establish it from scratch. I already have an appraisal form, but I am struggling with the calculations. I would greatly appreciate any expert advice you can provide.
