Performance appraisal form & calculation in an Import/Export company?

Dear Team, I am working in an Import/Export company, need to formulate a Performance appraisal.Kindly guide how should I start.
Here, there was no appraisal system set earlier. I have to set from the beginning. Need help!
Already have a appraisal form, but finding it difficult to do calculations. Need expert advice please guide.
