ESI facilities - affidavit stating husband dependent on employee

Dear All, Recently an employee has come to me stating that ESI dispensary needs an affidavit stating that her Husband is depending on her and his earning is Rs.5000/-or less PM. They have not obliged her case. If it is true, I feel that it is absurd . Request some one to clarify the correct position on this issue.

In the case of spouse there is no need to give an affidavit or declaration that his/ her monthly income is not more than Rs 5000. This declaration (online declaration is sufficient)is required only for dependents like father, mother or siblings who have no parents and are fully dependent on the income of the IP
