EPS contribution not remitted - PF asking for clarification letter?

Dear Sir, We are paid to employee how much we want to payable, But this amount is not separate as EPF contribution and EPS contribution while filing , Now that person's claim has been returned, as eps contribution not remitted this may be clarified then i want to pf office asked the same, they told give the clarification letter, Now my problem is how i can give this letter? Pls help me.

use form 3A and form 6 to fill the details and submit to office with a covering letter
