Struggling with EPS Contribution? How to Draft a Clarification Letter for PF Office

Dear Sir,

We are paid to the employee how much we want to be payable. However, this amount is not separated as EPF contribution and EPS contribution while filing. Now, that person's claim has been returned as EPS contribution was not remitted. This issue needs clarification. When I asked the PF office the same, they told me to provide a clarification letter. Now, my problem is how I can draft this letter.

To properly fill in the details and submit them to the office, please use Form 3A and Form 6 along with a covering letter.
