PF scheme certificate lost in transit - how to get duplicate scheme certificate?

Dear sir, Please note my Pension scheme certificate lost in transit.( certificate sent to my native address, cochin, kerala, but we shifted to chennai)
I badly need your valuable guidance to get duplicate scheme certificate. due to this issue my pension not yet started..

make a police complaint of lost certificate and visit the issuing office with your EPF details, they will ask to apply for duplicate, do the same with police GD copy, EPF will help to get your certificate soon
