Salary Increment Proper Procedure - increase employees 5% salary as an annual base

Dears HR Professionals, I am going to increase employees 5% salary as an annual base but i don't know about the proper procedure, we have annual performance appraisal which has 50 total scores but i want to know how to increase salary. kindly share your useful comments with me what i do?
