Hi Chitrali,
Answers specifically to your question is :

You will get 100% of average daily salary
2. Who will responsible to pay my salary my employer or ESI?
ESIC will directly credit you salary in your account. They will seek your account details along with medical slip issued by doctor for proceeding on leave. That you have to submit at ESI office of your area.
3. Can i reimburse my hospital bill of a private hospital which is not covered under ESI?
No, unless you are not referred to private hospital by the ESI doctors, no claim of reimbursements will be entertained.
