Seeking Joint Declaration Format for EPS95 Pension Benefits Beyond Wage Limits – Any Guidance?

Could someone please share a proforma of a joint declaration (employee and employer) allowing members of EPS95 the benefit of the actual salary in the pension fund exceeding the wages limit of either Rs. 5000/- or Rs. 6500 per month from the effective date as per the Honorable Supreme Court's order?
