Need Help Creating a Performance Appraisal Questionnaire for My Thesis Research

Hello friends , I'm working on a thesis for the Bachelor of Business Administration. My research topic is "Improving the performance appraisal at X company." I need a questionnaire about performance appraisal for employees and possibly interviews for employers. I have searched through some threads but can't find anything related. Please help me and reply to me as soon as possible 

FYI: My theoretical background includes the performance appraisal process, criteria, methods, frequency, who should evaluate, and factors affecting performance appraisal at the company.
