Is it compulsory to attach form 15g/h along with PF Claim form?

Hello All, The New Composite Form of EPFO has been introduced.
But can anyone tell me regarding Form 15G/H. Is it compulsory to attach form 15g/h along with PF Claim form and was is the purpose of Form 15g/h..

If the withdrawl amount is more than Rs.50,000/-,you have to attach the Form 15G/H to avoid Income Tax.
