Stuck with PF Withdrawal: How to Resolve Employer's Double Contribution Issue?

Hi, I had applied for PF withdrawal and was in process for 4 months. Now, I can see that it has been rejected because Form 19 & 10C have been rejected for the following reasons:

"Remarks by DA/AO: 12/2016 PAID IN 01/2017 CONTRIBUTION REMITTED TWICE BY EMPLOYER. HENCE UNABLE TO PROCESS"

I have asked my company representative about this. However, they are unaware of the error and unable to provide a solution to me. Can some HR expert answer how to solve the problem?

Please download your latest EPF Passbook to identify any discrepancies and share the same with the PF Office/Ex-Employer for the processing of your claim forms.
