PF Claim - CONTRIBUTION REMITTED TWICE BY EMPLOYER. HENCE UNABLE TO PROCESS?

Hi, I had applied for PF withdrawl and was in process for 4 months, now I can see that it has been rejected because Form19 & 10C have been rejected for the below reasons.
"Remarks by DA/AO:12/2016 PAID IN 01/2017 CONTRIBUTION REMITTED TWICE BY EMPLOYER. HENCE UNABLE TO PROCESS"
I have asked my company representative about this. But, they are unaware of the error and unable to provide solution to me. Can some HR expert answer how to solve the problem?

Dear Kumaran, Please download your latest EPF Pass Book to know discrepancies, if any & share the same with PF Office/Ex Employer for processing of your claim Forms
