Struggling with Apprentices Policy Approval: How Do I Create a Training Policy?

In June 2017, our company registered online with the Board of Apprentices Training. Last month, the Assistant Director from the Board inspected our company's infrastructure and plant. However, they have not yet approved it. Yesterday, I visited the Apprentice office, and they requested the following documents:

1. Apprentices Training Policy
2. Number of employees our company can hire after the apprentices training
3. Training procedures
4. Training evaluation method

We do not have an Apprentices policy, and without submitting it, they will not approve. Please provide the format for the Apprentices policy.
