Hi,

The ID card format is available freely anywhere on the web or with vendors. Check any template and customize it for your organization. The basic details needed, as mentioned in the format sent by a colleague, are as follows:

1. Name
2. Department
3. Designation
4. Company logo and contact address
5. Authorized signatory

You can also include blood group and alternative contact number if required.

If you refer to the Shops and Establishment Act, the details of the identity card and the information needed to be given on the card are adequate.

Section 17 Maharashtra Shops and Establishment Act 2017

Employer to furnish identity card to worker. The employer of an establishment shall furnish to every worker an identity card, which shall be produced by the worker on demand by the Facilitator. Such a card shall contain the following and such other particulars as may be prescribed, namely:

(a) the name of the employer;
(b) the name, if any, and the postal address of the establishment;
(c) the name and age of the worker;
(d) date of joining, department, nature of work, designation;
(e) the signature (with date) of the employer or manager;
(f) Blood Group;
(g) Aadhaar Card Number.
