Pension Calculation of EPS as per rules amended in Sept 2014?

Sir, Please read my attached filed and check my calculation as per rule amended by EPFO in Sept 2014 in regards to minimum salary for deduction is Rs. 15000/-

My daughter was serving with a public limited company and her contribution to pension was 541.00 and from 1.10.2014, it was Rs.1250.00. She retired from Company on 08.01.2018 and the total contribution to pension was Rs.115145.00. Her date of birth is 16.12.1979.
May I be please be enlightened how much pension she will be entitled and from when? What are the formalities to be completed?
