Same UAN continued in new company - PF withdrawal says data does not match

If an employee has worked 4 months in a company and continuing the same UAN in the next company for seven months when he was trying to apply online claim it shows multiple member id and transfer, when am trying to transfer it displays data not match even when I've given right details. may i know the solution?

1) download the passbook of both IDs and uan card from member portal
2) check the diff, as far as the mismatch, missing details may be reason,
3) if you are unable to resolve the issue, then use physical form 13 duly signed by present employer,
