Documents required for Medical reimbursement from ESI?

Sir I m a private employee and is taking esi facilities. I'm suffering from kidney problem and undergoing treatment from pgi. I wanted to undergo for claim of amount of treatment. In pgi they are asking for some form A and B. While in my hospital they are asking to write in plain page and telling me that form A or B is for govt employees. Kindly clear my doubt and which document is needed plz send me that.

I am sorry but, i am not able to understand your query. ESIc refers the IP's to private hospitals if ESIC hospital couldn't provide required treatment. were you reffered to pvt. hospital by esic. or you went on your own and now seeking for reimbursement from esic? forms you are talking about are of IRDA required to be filled by any insured to claim the expenses. form A is to be filled by insured and form B by the treating doctor. I couldn't find the link even in my answers but hope this solves your query.

sir, i want to know how i can take our medical reimbursement we take medical treatment from private hospital behalf of myself due to my kidney was not working that time and ESI were delaying to refer at that time i have take a treatment around 2.5 lacs please advice how to take reimbursement.
