EPF registration name in case of proprietorship firm?

I have a firm proprietorship , i registered it for epf registration but after registration, in registration certificate i have my own name instead of the firm, firm has not own any pan.
How could i get name of my firm on registration certificate.

Fill form 5A from unified portal user id and password , in the name field mention name of firm and in pan your pan card, in ownership it should be proprietorship , then submit.
New form 5A will be generated , seal and sign , submit to concerned epfo office
