Family member addition - daughter of 1 month and not enrolled in ESIC?

Sir, How add my family member in esic. I have daughter of 1 month and not enrolled in esic. She is suffering from a disease and admitted in private hospital.
Can i enroll her name in esic and can i obtain her treatment bill from esic. Kindly guide me.
