Latest norms and forms to be filled in ESI and PF?

Hi all,
Can you please help me out in latest norms and forms to be filled in ESI and PF.

For EPF the following forms need to be maintained
Form 11 (Declaration Form) Manual
Form 2 (Nomination Form) Manual
For ESI the following forms need to be maintained
Nomination Form Online
Remaining are all online submission only
