Lost Company Credentials: Do I Need a Specific Format for Form 9A Request?

Hi, I have lost our company user ID and password. When I submit the letter for a new user ID and password, the department asked me to submit Form 9A. I know that in Bangalore, the department issues Form 9A book. My question is, if there is any specific format needed to be filled for collecting Form 9A with a covering letter, or if only a covering letter with the company seal and signature is sufficient. Also, please share the covering letter format.

What is this Form 9A? Which department is asking for the same? You say you have lost the company user ID and password belonging to which portal. There is an option for "Forgot Password"; you can send an email from your official email ID.

There is no Form 9A in PF; it is Form 5A, the return of ownership.
