Institute EPF registration - document required and who to ask?

3 months ago i joined Institute. this institute not registered in EPF so here they says u have to take initiate for EFP Registration process.
so my question is
What r all document required in EPF registration and who to ask for registrar and write a letter to registrar?
please guide me and send me one sample letter

Dear Mr Raghu, First make a List of Documents you have readily.Log in to EPF PORTAL-Online Registration wherein you will find required documents for completing Registration process.you have been in the institute from the past 3 Months.This is sufficient to move forward.
