Esic cash benefit for my daughter's (age 1.5 years) orthopedic surgery

सभी सीनियर मेम्बरों को मेरा नमस्कार।

मेरा नाम सुमंत है, ओर में पिछले सन 2004 से रायपुर, छत्तीसगढ़ में एक प्राइवेट फर्म में काम कर रहा हूँ। यह फर्म में कर्मचारियों को ESIC की फैसिलिटी दी गयी है।

सन 2014 तक मेरा कॉन्ट्रिब्यूशन ESIC को जा रहा था। फिर अक्टूबर 2014 के बाद मेरा सैलरी 15000 से ऊपर होने के कारण मेरा contribution जाना बंद हो गया।

इस जनुअरी 2017 को जब से ESIC का नया सर्कुलर release हुआ कि, 21000 से कम सभी employee को फिर से esic का लाभ मिलेगा तो मेरा नाम भी इसमें शामिल हो गया कारण मेरी सैलरी अभी 19000 है।

अप्रैल 2004 से ले कर आज तक मे एक ही कंपनी में काम कर रहा हूँ। सैलरी स्लैब के कारण में पिछले 3 साल से esic से बाहर हो गया था।

में अभी मेरी लड़की (उम्र 1.5 साल) का पैर का ऑरोथपेडिक सर्जरी बॉम्बे में एक प्राइवेट हॉस्पिटल में करना चाह रहा हूँ, जो कि congential प्रॉब्लम हैं।

इस साल जनुअरी 2017 से मेरा ESIC कॉन्ट्रिब्यूशन चालू हुआ है। क्या किसी स्पेशल लॉ की तहत मुझे ESIC से कोई लाभ मिल पायेगा। क्यों कि इस आपरेशन में डॉक्टर के कहे अनुसार 4 से 6 लाख तक कि खर्च आ सकते है।

I have shared your employment and contribution details.It is found that contribution from January, 2017 to April, 2017 has been made.Your family can be availed superficiality facility if your contribution is made minimum 178 days or 6 months.So after June if contribution made then your daughter can avail this benefit.

Please note the following

1. Enroll your daughter name(In family details) in the ESIC portal and generate TIC.

2. You can approach registered dispensary along with your TIC to avail these facility.

3. Request and convince them to refer your case to their tied up private hospital.

I have already enrolled my daughter's name in my family details and got TIC Printout from my employer. Sir this is request you to please follow the circular Ref. 11/1/1/96 dated 30/05/2017 released from ESIC Department.
Perhaps, It shows any path.
