Questionnaire for testing staff cafeteria food quality in Hospitality industry

Dear All,
Need you advice on framing questionnaire on testing the food quality served for staff in cafeteria, for which we maintain a Food testing register so that timely improvement can be done.
So need few parameters for framing questions which will open up employees view point in terms of food taste and quality
