Steps to Print the Submitted Accident Report
The following are the steps to take a printout of the submitted accident report:
1. Log on to your ESIC portal - Accident report (form 12).
2. Insert the IP number.
3. Select "Modify Last Report" in the "Do you want to select" column.
4. Click "Submit."
5. You will receive an error message stating, "Currently, modification approval is not provided"; click "OK."
6. You will see the IP details again; click "Cancel."
7. You will be prompted to upload certificates for benefits.
8. Select "Accident Report Mapping."
9. Fill in the insurance number.
10. Click "Search."
11. Search again.
12. You will see a checkbox to map the Accident report.
13. Select the reference number.
14. Print the Accident report.
