Healthcare Start-up- Recruitment Of Doctors As Consultant Or On Fixed Contract?

I am setting up a healthcare start-up who will be engaged for O&M of remote occupational health center of Industry.
Want to recruit doctors.They are required to provide their services 3 days a week ( part-time category)
Q1. Advise if it should be as consultant or fixed term contract?
Q2.In case of fixed contract, do we need to provide salary break-up in Basic DA and other allowances? or a consolidated amount?
Q3. In either case can someone provide me an appointment letter tailor made for the doctors?
