Procedure Or Form Required By The Cooperative Societies

I want to know which form or documents would be required to present to an employer in order to deduct the salary to meet the dues?
I am new to this field and right now working with a cooperative society.
I know that every state would have different procedure for the same but is there any way to extract that info or is there anything common for this payment of dues?

For income tax or TDS deduction it is form 12BB
For other deduction if salary is 18000 or below then forms and returns under payment of wages act
For welfare deduction there are different return in different state.
