Assistance Required for FNF and PF Claim Process

One of my staff left the job after two years with proper resignation and notice period. His Full and Final (FNF) settlement couldn't happen in the next cycle, and before the next cycle, he committed suicide after his brother's road accident death. He doesn't have any dependents as he is unmarried. He has a father, mother, and two sisters in his family. As per company records, his mother is his nominee.

Now, I want to help his mother claim all the FNF settlement amount, Provident Fund (PF) amount, and any other things that can be claimed. My HR said she needs an affidavit and a letter to the company along with the KYC of his mother and account details, along with the death certificate. I have managed other things. However, she is unable to get the formats. I need the format of the affidavit and a letter to the company. Please help me with this.

My HR is unable to get the formats. I need help for my HR and my staff's mother to get the claim ASAP. The mother needs the money urgently as both of her earning sons have passed away.

Affidavit Draft for Claim Settlement
