How Can HR Fairly Evaluate Employee Performance When HOD Disagrees?

Respected Senior, I require your valuable advice on the matter of performance appraisal. In our company, one employee joined on 07-July-2016. As per his appointment letter, his confirmation date was 06-Jan-2017. However, his HOD extended his confirmation for the next six months. According to his personal record, he always comes on time, does not take much leave, and we have not received any complaints against him. However, his HOD found his performance unsatisfactory. My questions are as follows:

Q: 01) As HR, how can we identify his performance?

Q: 02) What kind of parameters can HR use to evaluate his performance?

Q: 03) Sometimes disputes arise between the HOD and employees. Should we trust the HOD in such cases?

I need this on an urgent basis.

If performance was not up to par, the shortcomings should have been discussed and recorded, at the very least, discussed. Unless the performance evaluation parameters are known to employees, how can they work towards achieving them.

Employee is thoroughly knowledgeable about the performance evaluation parameters. We have discussed this with the employee as well as the Head of Department (HOD), but I am looking for another way to assess his performance. For example, when we inquired about his behavior with other department heads, they did not have any complaints against him. Therefore, I would like to understand how we can evaluate the employee's performance.

The performance review/appraisal is not available as a tailor-made solution. It should be designed to suit one's nature of work assignments, timeline, and many other parameters. Simply making it available doesn't serve the purpose.
