Next Step After Confirmation On Pf Withdrawl Rejection

I applied for pf withdrawl on 7 november (slip available)
I just check few minutes back and got a update on the portal stating pf withdrawl rejected due to mis match in joining / last date of working as per last employer
So need to know what is the next step on the same

Mr.Jai,
You may submit in concerned pf office form 5 and form 10 ( revised) duly signed by your previous employer which should be corrected /match with your pf records along with request letter giving reference of portal rejection status
