Although a printable Form 13 is available for download, the PDF file is blank when I attempt to download it. Could you please advise on what steps I should take next?
Today (November 29, 2016), my claim was rejected as the hard copy of the claim form was not submitted. Every time I try to download the printable Form 13 and open it, it appears blank. How can I re-apply for the "Claim Transfer"? Why is the printable Form 13 empty upon download, and how can this issue be rectified? Is there a specific timeframe within which I must re-apply for the transfer?
