EPF Claim - Necessary Enclosures?

What are the necessary enclosures must need to be attach with EPS 10D, EPS 10c, EPF form 19 while submit for claim.

While forwarding the claim to pf office, it is better to attach the Form5,Form10,Form3a (for the last period of the claimant) and Break particular sheet for the sake of our ex-employee and though they are not specifying such attachments, it is to make easier to them for speeding up the claim.
