Please tell how to fill ESIC accident report. its urgent

frist write accident book, TWO wtness is required,
then log in to ESIC portal and submit the details of accident book there, take print out

Take a screenshot (PrtScr) as of now or else use the CTRL + P. And inform the same to the ESIC authorities,

I was trying to submit the Accident report in the ESIC online portal. But not able to submit and getting Server Internal Error or else Secure Connection Failed. Can you help me out on this issue.
