Write A Final Warning Letter To An Employee For His Unauthorized Absences

Dear All,
Please guide me to write a final warning letter to an employee for his unauthorized absences and he promised us and he has given us a statement saying that he will not repeat similar behaviors again. Also this is not the first incident and in the first incident also he gave us a statement and said that he will not repeat such behavior and he did the same thing for the second time. we gave him a chance for the first time but not for this time. please help me.
