Navigating PF and Pension Claims After an Employee's Natural Death: Which Forms and Documents Are Needed?

Dear All, I am currently working in Gujarat Circle Company, and I have a query about PF withdrawal. The details are as follows:

An employee, who has been working with our organization for the last 30 years, passed away on 20.09.16 outside company premises due to natural causes. Please advise on which forms to fill out for PF withdrawal (Form 20) and Pension (Form 10D), and provide guidance on the procedures related to forms such as gratuity. Additionally, could you specify which documents are required from EPFO and ESIC for pension and PF amount withdrawal?

Kindly suggest the appropriate forms and necessary documents to be attached to each form.

Good Evening Karan,

These are the documents that have been submitted by you to the PF and ESIC local branch:

1. Death certificate for the employees.
2. Nominee ID Proof for those nominated in PF Form-2 revised.
3. Employee ID Proof.
4. Police FIR copy.
5. Muster Roll copy for employees.
6. All documents are attested by a company authorized person.

Please submit the following documents under EPF:
1) Form 10-D (widow/children pension)
2) Form 20 for Provident fund withdrawal
3) Form 5 (IF) for EDLI along with the original death certificate, 3 photos, Bank passbook Xerox copy, and family survival certificate.

If the deceased employee has minor children, submit a separate bank account for pension. Birth certificates should be attached with the above claim application.
