Sir, benefits under ESI Act, 1948 and rules/regulations framed thereunder are available for a certain period and that too on the conditions that the insured person must be entitled for the said benefits. The appropriate branch office of ESIC to which the said insured person is attached only can certify as to for which benefit the said insured person is entitled or not (even after exhausting 90 days as mentioned by you). Therefore, it will be more better if you may contact the said office. However, in order to see the terms and conditions of eligibility, please see online link of literature as available on website of ESIC:-

Mr Dharmender Singh,
If a workman is medically unfit and getting continuous treatment under ESIC, allowing him medical rest, he will remain eligible till completion of treatment (medical fitness) irrespective of his zero contribution, subject to his entitlement at the beginning of the treatment.
His entitlement will be checked when once he get fitness and again requires general sickness benefits. However, in case of injury during the course of discharging his duties, he will again be entitled.
