ESI registration - Is it necessary to register the shop & establishment?

Sir, I want information for ESI registration. One of the my friend doing a business for Crane service he want to register the ESI.
1. Number of employee or operator is 11 number.
2. He is registered a) DIC certificate and Service tax certificate
These document is enough for register or not ? Is it necessary to register the shop & establishment?
