ESI Registration for Crane Service Business: Do We Need to Register the Shop & Establishment?

I want information for ESI registration. One of my friends is running a Crane service business, and he wants to register for ESI.

1. The number of employees or operators is 11.
2. He is already registered with a) DIC certificate and Service tax certificate. Are these documents sufficient for registration? Is it necessary to register the shop & establishment?
