How Can We Create an Effective Employee Training Passport with Scores?

Can anybody help me with a sample training card? In our company, we want to develop a training passport which each employee will carry. This card will have all his training details with training effectiveness scores.

Dear Panthala,

Your TRAINING Passport can be styled like a normal passport with the cover carrying the Company Logo and the words "TRAINING PASSPORT" in the center of the cover. The first page should include the individual's personal information like NAME, DEPARTMENT, EMPLOYEE CODE, LOCATION, with the person's photograph. The rest of the booklet should include blank pages to be filled by the respective trainer with the necessary details like DATE, TOPIC, TRAINERS' ASSESSMENT.

- Function
- Employee code

The second page would include:
- Training Needs Identification (TNI)
- Name of program
- Name of trainer
- Date
- Signature
- Any other training programs attended
