Training Card [Thread 56607]

Can anybody help me with a sample training card.In our company we want to develop a training passport which each employee will carry and this card will have all his training details with training effectiveness scores.

Dear Panthala,
Your TRAINING Passport can be styled like a normal passport with the cover carrying the Company Logo and the words .......
.............................."TRAINING PASSPORT" in the centre of the cover.
The first page should include the individual's personal information like
NAME
DEPARTMENT
EMP CODE
LOCATION
with the person's photograph.
The rest of the booklet should include blank pages to be filled by the respective trainer with the necessary details like
DATE
TOPIC
TRAINERS' ASSESSMENT
