In order to help us suggest changes to your questionnaire, if any are required, kindly tell us how you will use the questionnaire. For example, whom will you administer the questionnaire to, and how? Depending upon the audience, you need to decide whether to include an option "Do not know" for some questions.

I am planning to use this questionnaire on employees of different companies. I have some friends in various companies and will also try to interview other employees for my project. I will surely review my questionnaire again and add a "Do not know" option if required. Can you please suggest if any changes are needed, or should I keep these same questions?
