Our employee passed away while in service in 2009. His spouse is seeking to claim the EPF in 2015. She reminded me about the claim she applied for, which has now come to the attention of the local EPF office. The officer in charge has requested records of attendance and salary. Unfortunately, the salary register is not available. Although the attendance register is available, it mistakenly shows attendance marked for him for about a month, indicating a lack of awareness. The salary details are present in the EPF statements. Due to these issues, the officer has asked for reasons to be provided with an affidavit. Could you please advise on how to draft an affidavit that addresses and satisfies the concerns raised by the EPF officer?

Nothing is required to be shown to the EPF department or concerned officer, but she is eligible for pension and ELDI benefits through EPF. She will receive pension and other benefits from the date of her husband's death. You may have records of his joining the provident fund and the date of his last contribution.
