New to HR: How Do I Break Down a 3 Lac Monthly Salary?

I am new in HR, and my director has asked me to create a salary breakdown for a monthly salary of 3 lac Rs. I am unsure about which salary components to include for this 3 lac Rs. monthly salary. Please assist me with this by providing a format for the same.

Is PF and ESIC liability there?

If yes, then:

Basic - 45% of CTC
HRA - 50% of Basic
Medical - 1250 (Fixed)
Allowances - 1600 (Fixed)
Special Allowances - PF - 12% of Basic
