Need Help with HR Audit Internship: How Do I Create a Questionnaire and Analyze Data?

I am doing my internship in HR at one of the reputed hospitals in Hyderabad. My topic is HR AUDIT. Can anyone please suggest to me how to prepare a questionnaire and what data is to be analyzed? It's my summer internship project. I request all to suggest to me regarding this. Thank you.
