How To Track Employee Productivity For IT Company Into Website Development

Hi,
So my manager has asked me to prepare a sheet wherein the top mgt. can track on what projects are running, which employees are working on it, how many projects has an employee worked on in a year, etc. that will help us to have a performance review based on the projects undertaken by him/her.
Please help me with a template in excel sheet.
