I have heard too much from my colleagues, and now I am in need of Form 5 & Form 10. Please provide me with details about these forms and also a copy of both forms.

You can find those from PF office as well. Ask for Form 5, 10, and 12A. Form 5 comes under The EPF Scheme, 1952 [(Para 36)(2)(a)] and The EPS Pension Scheme, 1995 [Para 20(4)]. It is to be sent with Form 2 to the commissioner.
