No need of any documents from the employees for registration. Initially you have to get the voluntary registration from both ESI & EPF for coverage. After the coverage you can collect some of the documents such as Form 11, Nomination forms, KYC Docs etc... for ESI & EPF

Sir, if a firm or factory is employing only 5 persons( as mentioned in subject head of this thread), then the same is not coverable under ESI Act, 1948. Coverage under said Act is not optional.

I think voluntary coverage under ESIC is allowed for companies with less than 20 employees.
Voluntary coverage for individual employees with salary above 15000 (Now 25000) is not allowed.

Compliance with pf and ESIC comes with a headache and cost both in terms of efforts and increased payroll cost.
Both the acts don't apply to you at the moment.
Most employees working for a startup don't care for them either
So why are you interested in registering for them?

Very True, If you do not fall under the category of compulsory PF, why invite troubles of Statutory Compliance and unnecessary troubles and harassment at the moment. Cross the bridge when it comes. Members can go for PPF or NPS voluntarily.

ESI is covered if the employee strength is 10 and above. It is not 20 and above. Till now ESI is contributed only on 15000/-(25000/- is not yet implemented. Discussions are going on and till now there is no any GO)

Hi Bharathiji,
Enrolement itself require lot of paper work, documentation & follow up with concerned Authorities. When U R in Startup Co. & U have enough of other urgent works to be attended, U can delay PF till it becomes mandatory.

Actually there is no provision. It is possible under EPF Act for epf.
