Looking For Sales Interview Questionnaire

Hello All,
I'm working for a product based company who has developed hospital management software and now looking to hire more sales guys from core sellers to city heads. Can some one of you guide me how to prepare a interview questionnaire with a score card to short list sales people across all levels.
