Is the Prescribed Form for Section 192(2D) Claims Officially Notified Yet?

The Finance Act 2015 introduced Section 192(2D) effective from June 1, 2015, concerning the prescribed form and manner for obtaining evidence, proof, or details from employees regarding specified claims (such as deductions under section 80C) for TDS computation. Has this prescribed form and manner been officially notified?

You can refer to CBDT Circular 20/2015 dated 02-02-2015 regarding TDS from Salaries. However, in that circular, no formats were prescribed. You can use the attached form for collecting details from employees.
