Is Form 5A the Final PF Certificate? Seeking Clarity and Examples from Karnataka

Hi all, I am new to this site. Is Form 5A the final PF certificate, or is the government providing any other certified certificate? If you have the PF certificate (Karnataka) latest one from 2016, please upload the image so that I can see how the original certificate looks.

You will receive the PF cover letter. No certificate will be given. You can even take a print of Form 5A.

Actually, my auditor is saying that in 2016, we will not receive any certificate from the government. Form 5A is final. Sir, if you have any latest PF certificate, please upload it so that I can understand the certificate format.
